Since the 1st century specific gemstones have been connected with the signs of the zodiac and have been incorporated into birthstone jewelry. Originally birthstone systems were based on the individual color of the stones. The origins of the practice are thought to be connected to the Breastplate of Aaron which incorporated gems associated with the twelve tribes of Israel. Each stone had the name of the tribe inscribed on it and they were sewn into the breastplate in four rows of 3. Though the names of the gems used are established, the precise translation is arguable so the specific stones that were used are unknown. This has led to various gems being associated with the different zodiac signs.

In later times birthstones became associated with the months of the year rather than the zodiac signs and there has been a lot of variation in the month to which a gemstone is assigned. The most commonly used system today is one which was proposed by the Association of Jewelers in America in 1912, and incorporates some substitutes for the higher-priced gemstones. One gemstone has been added since then. Tanzanite was added as a birthstone for December in 2002 by the American Gem Association though it is not acknowledged by everybody.

Alternative schemes which associate distinct gems with the birth month include Ayurvedic birthstones and mystical birthstones. The mystical birthstones system is thought to date to over ten centuries ago and originated Tibet, the Ayurvedic birthstones list originated in ancient Indian medicine and philosophy and is thought to date to approximately 1500 BC. However the majority use the modern lists initiated by the Association of Jewelers.
